We work closely with hospitals, families and discharge professionals to help people with a disability to exit hospital settings and re-enter the community and begin their journey towards finding long-term accommodation. We help people with a disability who are medically ready for discharge from hospital to transition back into their home, or into new or temporary accommodation funded by the NDIS while providing them with the supports appropriate for their needs. Lead community Support Services committed team will work with your and/or your loved ones to:

• Collaborate and manage the process of discharge and transitioning back into the community.

• Explore suitable housing options and/or arrange modifications where necessary. 

• Sources suitable Medium-Term Accommodation and coordinates a team of trained support workers to provide around-the-clock support with funding from the NDIS 
• We will work alongside you to develop person-centred support plans that meet your individual needs.
