Choice & Control is central to how we support you implement services and supports you receive in your plan. We will work closely with you and your support network to provide transparent advice about support options available, and respect your rights to freedom of expression, self-determination and decision-making. We will connect you with the supports you need to achieve your NDIS.
Our support coordinators will help you find and engage suitable service providers that help you to work towards and achieve your goals. These providers could be NDIS providers or community and mainstream providers.
We will ensure that you and your key supports understand what is contained in yours plan and how you can be supported to explore and identify appropriate informal, mainstream and community, and funded supports as required.

Our friendly team will help you:
-Implement your NDIS funded support so that you get the best out of your plan.
-Assist you with accessing community and mainstream services.
-Provide assistance with accessing disability-specific services.
-Provide assistance with accessing community and mainstream services.
-Support you navigate NDIS pathways.
-Build your confidence and skills to use and coordinate your supports.

CONFLICT OF INTEREST:
As a provider, we have an obligation to put processes in place to limit conflicts of interest where possible.
Our organisation has policies and procedures that outlines how we reduce the risk of conflicts of interest.
In summary:
We reduce the risks of conflict of interest by separating our service delivery team and the support coordination team. -Ensure that information and records of supports participants have received remain confidential;
-We ensure that participants receive all the information regarding support options under the NDIS;
-We ensure that participants are aware of their rights to choose support from a provider that is different from us.

At Lead Community Support is a NDIS Registered Services provider , we firmly believe that a person’s disability should not impede rights to choose, or hinder achieving goals in life
